-Copays and prior patient balances are to be collected prior to the time of service
-We require a $50 deposit from patients who have not met their deductible
-Our office may charge $25 for any no show or cancellations less than 24 hours
-If you show up late, we reserve the right to reschedule your appointment. We will do our best to accomodate you but this may cause longer wait times.
-For any new medical concern or new medication request; patients must schedule an appointment. The healthcare provider cannot simply just call in a medication.
-Forms: FMLA, work or disability forms can only be filled out during a designated appt for that form. We cannot fill out these forms during routine wellness or a sick visit if it is unrelated.
-Refill request may take up to 2 business days. They may require an office visit if medically indicated. We do not provide routine medication refills on weekends. Patients are subject to a $25 fee for any medications prescribed on the weekend.
-All lab orders and imaging orders will be automatically cancelled if not completed within a certain time frame. After such time, patients will be required to have a return visit in order to obtain another order form.
-Any new STD testing must be seen on site first.
-We do not accept any urine or stool specimens from patients without a scheduled appointment.
-Annual wellness visits (Physicals) do not cover additional medical concerns such as symptoms or refills of medication. You are subject to your copay or rescheduling for those concerns.
